Discussion Agreement for
Allegations of Academic Integrity Violations
(Option A)

This form is to be used in assessing academic sanctions for violations of academic integrity. If the instructor and student agree on a sanction (e.g., reduction in course grade, failing course grade, assignment of additional work), no disciplinary action will be taken by the Provost's 0ffice as a result of this specific act alone. If, however, this specific act represents the student's second or greater instance of academic dishonesty, the Provost's office may choose disciplinary sanctions (e.g., suspension, dismissal) in accordance with the Collected Rules section 200.010.

We fully understand that by the student's acceptance of this academic sanction he/she knowingly takes full responsibility for the incident, admits that he/she violated the University's academic integrity policy, waives his/her right to a formal hearing, and agrees with the proposed academic determination. The Office of the Provost will maintain a record of this violation and may assign appropriate disciplinary sanctions for additional instances of academic dishonesty.

We further understand that the student has the right to rescind this agreement by notifying the Office of the Provost within 5 working days from the date this form is signed. If he/she chooses to exercise this right, he/she understands that a hearing will be convened according to the guidelines found in the Collected Rules, section 200.010, to determine if academic dishonesty occurred and if so, to assign the appropriate sanction for the violation. In addition, we understand that if the student fails to comply with the penalty listed above, the Office of the Provost may convene a hearing to impose disciplinary penalties.
